Alex (Arnold Schwarzenegger) and Angela (Pamela Reed) successfully deliver their respective babies -Alex, a girl; Angela, a boy- and everyone lives happily ever after.

After being at odds about whether to continue the pregnancy experiment, both Alex and his colleague Larry (Danny DeVito) agree to carry on with it. Alex finds himself experiencing what appears to be a normal, healthy pregnancy, with all possible symptoms in tow -morning sickness, mood swings, soreness in his nipples, etc.-, while Larry struggles to keep the whole affair a secret from Dr. Banes (Frank Langella), the Laboratory Director.

Parallel to this, both men find themselves in respective relationships: Alex falls for his colleague -and contributor of the ovum that made possible for him to “get pregnant”, along with the experimental drug- Diana (Emma Thompson), while Larry seems to be rekindling his relationship with his ex-wife Angela -who, to his shock, is now also pregnant, from a one-night stand with Aerosmith’s personal trainer-.

Eventually, Alex’s pregnancy is discovered and Diana is infuriated when she figures out her “Junior” ovum was the one used for it, leading to a fight between them. At the same time, Dr. Banes finds out and goes to great lengths to take credit for the whole thing. To avoid public scrutiny, Alex takes refuge in an expecting mothers’ private retreat and justifies his masculine aspect due to “being a bodybuilder and using too many steroids in the past”. Eventually, Diana comes to visit him and they both reconcile, when they come to terms with the fact that they can and want to be parents to their future child together.

Larry, in turn, wanting to derail Banes’ ambitions, shares his data with rival company Lyndon Pharmaceutical and they are enthusiastic about a partnership and prospective production. Time passes and Alex finds himself eventually experiencing what seem to be labour pains, for which he reaches out to Larry and Diana; Diana rescues Alex from the retreat, and Larry secretly talks to a colleague to prepare an operating room for an emergency C-section. However, a janitor catches wind of this conversation and alerts Baines.

As Larry and Alex arrive in the hospital, they discover a plethora of journalists standing by; it is revealed that Banes summoned the media, aiming to intercept Alex on arrival and receive full credit and praise for “being the force behind the world’s first pregnant man”. However, before they are both caught, Larry’s colleague warns him and they devise an alternate plan: Larry seeks the help of Angela, whose pregnancy has followed the same term as Alex’s, to bring HER to the hospital’s front doors, which causes a scandal and an embarrassment against Banes, getting him discredited and fired; all the while, Alex and Diana are snuck in through the fire escape.

Alex is taken away for his emergency surgery, where Larry and the colleague determine it is turning into a high-risk C-section, as the baby appears to be stuck on a delicate upper corner of Alex’s large intestine. Meanwhile, Diana stays behind to keep company of Angela, only to see her starting to experience labour pains as well and coach her through the process.

Eventually, Alex successfully delivers a healthy baby girl. Larry runs to announce it to Diana and finds her assisting Angela in delivering her own baby; they swap places. Diana rushes to Alex and they gleefully reassure their love and name their daughter “Junior”, while Larry and Angela deliver, together, what turns out to be a boy, whom they call “Jake” and reconcile to raise him together.

The plot forwards to a year later, and the two happy couples are celebrating, on vacation by the sea, the first birthday of little Junior and Jake. We find out that Diana became pregnant with their second child and Angela expresses the desire to also have a second child with Larry, but struggles to convince Larry to do what Alex did and carry it himself, to which he comically refuses. The movie ends with Alex chasing after Larry, attempting to convince him to agree to a pregnancy.
