Everyone who should have died in the roller coaster incident at the beginning of the film does, in fact, eventually die.
Long Ending:
High school yearbook photographer and graduating senior, Wendy Christensen (Mary Elizabeth Winstead), snaps photos of various classmates at an amusement park. Her boyfriend, best friend, and best friend’s boyfriend Kevin (Ryan Merriman) coax Wendy into going on a Devil-themed roller coaster (with a cameo voiceover by Tony Todd!). She is seated with Kevin, and then has a vision that the roller coaster will crash and kill everyone aboard. Wendy freaks out and gets off the coaster, as does Kevin, jock Lewis (Texas Romero), goth couple Ian (Kris Lemche) and Erin (Alexz Johnson), friends Ashley (Chelan Simmons) and Ashlyn (Yan-Kay Crystal Lowe), perverted jerk Frankie (Sam Easton), and two people who Wendy didn’t see the identities of. Nobody else is allowed off the coaster, and the staff launches it. As Wendy foresaw, the coaster crashes and kills all aboard (including her boyfriend and best friend). And now that they’ve survived death, the spirit of death begins pursuing the survivors to kill them as well. It begins hunting them, planning to kill them in the order they would have died in the roller coaster crash.
Wendy struggles with survivor’s guilt, confiding to her younger sister Julie (Amanda Crew) that she blames herself for the death of the others. Wendy finally begins reviewing the pictures from that night. A picture of her boyfriend has the roller coaster he was killed on prominently in the background. Wendy begins suspecting that the pictures hold clues to the survivors’ future deaths. This is confirmed later when Ashley and Ashlyn are killed inside of malfunctioning tanning beds. In their picture, they were posing with tropical plants (i.e., where you’d get a tan).
At the funerals of Ashley and Ashlyn, Wendy tries to convince Kevin that death is trying to kill them and that her photographs contain clues as to what will kill them. Kevin is skeptical, but accepts Wendy’s claims after he personally witnesses Frankie get killed in a freak car accident when a car’s motor fan chops off Frankie’s head (and his picture had him posing in front of a fan). The pair then try to save the next three in line for death’s revenge, but only manage to save Ian. Lewis and Erin are killed by death in ways forecasted by Wendy’s photographs.
The unidentified passengers are next in line (and then Wendy and Kevin are the last ones on death’s list). Kevin goes to his job as security at an Independence Day celebration while Wendy looks for clues as to who the unknown passengers are. She recognizes the bracelet of one of the unknown passengers as belonging to her sister Julie. Now knowing the other passengers are Julie and Julie’s friend (both of whom are at the Independence Day celebration), she calls Kevin. Based on the pictures, Julie and her friend will be killed by horses, Kevin will be killed in an explosion (Kevin uneasily eyes the celebratory fireworks as Wendy reveals this to him), and she is to be killed by Ian. Wendy then rushes to the celebration to help find her sister. Though Kevin is able to save Julie from a runaway horse, Julie’s friend is not so lucky and is killed. Wendy then manages to pull Kevin away from a grill just before it explodes, saving him. At that moment, Ian arrives (blaming Wendy for Erin’s death). Meanwhile, fireworks that have been knocked over in the pandemonium shoot at Wendy. Julie and Kevin push Wendy out of the way, and Ian is killed in the explosion. The trio celebrates surviving death a second time.
Months later, Wendy has moved into the city and is on a subway with new friends. Unexpectedly, Julie (who is paying Wendy a surprise visit) gets on the same subway car. Wendy grows more uneasy when they find that Kevin is also on the subway car (independently visiting the city). Wendy then has a vision of a horrific subway crash in which everyone dies. Unfortunately, the vision arrives too late. There is no way off the train, the emergency stop cord malfunctions, and we hear the train crash and the main characters scream as the screen fades to black.
Order of Deaths:
- Roller coaster patrons – As the roller coaster crashes in spectacular fashion, limbs are lopped off, certain carts get loose and crush those strapped in, and others fall out of their seats when the restraints disengage.
- Ashley and Ashlyn – They get into tanning beds located next to each other. The temperatures begin to malfunction after death causes Ashley’s soda to leak onto the temperature controls. The two girls start getting burned as the heat continually increases. They try to get out, but a shelf collapses and pins down the lids of the tanning beds. The two women are flame-broiled to death inside the tanning beds.
- Frankie – While in a drive-thru, a runaway truck collides into Kevin’s car (Kevin and Wendy manage to get out just before the collision). The crash ejects the car’s engine out of the hood and into the car in front of theirs, which turns out to be Frankie’s. The still-rotating car engine fan chops Frankie’s head to pieces.
- Lewis – While exercising on a shoulder press machine located under the school’s crest, the machine knocks loose two sabers that are part of the crest. The sabers miss Lewis, who now believes he is safe. He does one last rep to celebrate… and has his head crushed in by the collapsing weight machine (the sabers had actually cut the wires suspending the weights).
- Erin – Death manipulates a forklift to go rogue at the hardware store where Erin and Ian work. The forklift knocks over a shelf that has various posts on it (which were meant to kill Ian). Wendy knocks Ian out of the way, saving his life. However, the shelf collapse causes Erin to slip. She falls headfirst onto a nail gun, which launches a series of nails into Erin’s skull.
- Julie’s Friend – A runaway horse that is scared by the fireworks breaks loose and begins running wild through the Independence Day celebration. Julie gets caught on a rope tied to the horse and is almost dragged to death. Kevin saves Julie, and authorities wrangle the horse. As the horse is bucking, the rope attached to it snaps a flag pole in half and sends the pole flying. The pole fatally impales Julie’s friend.
- Ian – After fireworks explode on the ground, they knock over a crane. The crane arm smashes Ian.
- Julie – As the subway car she’s in crashes, debris tears through the car and smashes Julie.
- Kevin – The subway car skids onto its side and Kevin is thrown through a window, where he is ground to death as the subway scrapes him against the walls of the station.
- Wendy – She survives the crash, but her legs are broken as she’s thrown from the subway car. Wendy realizes she has landed on another set of subway tracks. She is then struck and killed by a second subway car.
